Change Makers Check In Form

Here is the link to the Change Makers Dashboard.  Fill this out as often as you like, ideally at least once a month (as per the email reminder).

Remember to share your completed form with your mentor so the next time you meet they’re across what you’re up to.
You can do this by:  saving your answers before clicking submit: right-click anywhere on the form, select print, then use the ‘Destination’ dropdown to ‘save as PDF’. You can then send them the PDF via email.
